Ordering Fascian™ or Fasciablast™
Ordering from us is a simple process. You will need the following information handy when placing an order:
- A copy of the physician's medical license, sent to us via fax (not required if we have it on file)
- Your shipping and billing addresses
- Quantities and sizes of the products you wish to order
- Shipping method desired, or the date you need our products
- Method of payment
Order forms are available and may be faxed to us.
We ship our products via UPS and welcome overseas orders. Shipping and handling apply to all orders. Ask us for current pricing information or if your order needs special handling. Please note that we can only ship our products to licensed physicians.
